Creating Meaningful Outcomes

Last year, the OIG identified a trend in Home Health regarding underreported falls with major injury and hospitalizations.  Home Health Value-Based Payments also emphasize the impact of quality outcomes based on accurate reporting of behaviors, dyspnea and GG functional OASIS items.  Join us as industry expert and physical therapist Cindy Krafft connects the dots between these industry challenges and trends to help home health providers attain optimal outcomes based on intentional care planning and accurate OASIS data collection which ultimately impact agency HHVBP outcomes and STAR ratings. In addition, falls prevention best practice strategies and documentation will be shared to address and help mitigate the trends identified through the OIG. 

Objectives

  1. How to attain optimal outcomes through intentional care planning based and accurate data collection.
  2. The impact of quality outcomes based on reporting behaviors including current HHVBP [dyspnea] and 2025 HHVBP [GG functional items].
  3. The importance of accurate falls data collection, documentation, care planning and reporting along with best practices to mitigate falls with injury.

Cindy Krafft PT, MS, HCS-O

Cindy Krafft PT, MS, HCS-O is an owner of K&K Health Care Solutions based in Florida. She brings 30 years of home health expertise that ranges from direct patient care to operational / management issues as well as a passion for understanding regulations. Years spent in the direct patient environment solidified that this was the optimal setting for the provision of effective and efficient interdisciplinary care as it focuses on functionality and patient centered care delivery. Recognition that the providing care in the home environment presents both unique opportunities and challenges is clearly evident in both training and consultation activities.

For the past nearly 20 years, Cindy has been a nationally recognized educator in the areas of documentation, regulation, therapy utilization and OASIS. She has and currently serves on multiple Technical Expert Panels with CMS Contractors working on clinical and payment reforms and bundled payment care initiatives. Her focus is on providing the knowledge and tools critical to the post-acute continuum of care to operationalize external requirements while keeping the driver of care where it needs to be – the needs of each patient being cared for.
