Education Overview

Dew-Anne Langcaon, of iHealthHome and Ho’okele Health will be presenting education session, and this entire Care Management series which will be broadcasting every other month.  In this particular webinar, she reviews the typical assessments, client information, reports and other processes used by care managers in delivering their services and address how to unlock the hidden data elements captured through routine workflows and transform them in to valuable metrics to demonstrate value and return on investment in care management.

We will provide a framework for care managers and in-home providers to identify, measure and communicate the value of their services to clients through quantifiable metrics.

The specific Learning Objectives for this webinar are:

  • Identify key process and outcomes metrics that define the value of care management services.
  • Use a framework to collect the data, analyze and present metrics.
  • Communicating metrics to create understanding of the value of care management.
