In 2016, the Centers for Medicare & Medicaid Services (CMS) initiated a demonstration program testing out the impact of a value-based purchasing program in Medicare home health services. With just over a year of experience in home health value-based purchasing (HHVBP), home health agencies need to evaluate what are the early outcomes and what best practices have emerged to improve HHVBP scores and qualify for bonus payments. This program is the second phase of the educational effort by IAHC and the National Association for Home Care & Hospice to prepare home health care leaders for the brave new world of value-based payment.

The program combines an early data analysis of the changes in HHVBP measured outcomes with real life successes by clinicians and financial operations professionals that show the pathways to improved patient outcomes, higher achievement scores and a winning year in HHVBP.

About the Speaker

Diane Link

Diane Link has been a RN for over 25 years with over 24 years of home health and hospice experience.   She is currently a senior clinical consultant with BlackTree Healthcare Consultants specializing in all aspects of clinical management including outcome improvement programs, regulatory and compliance programs, episodic management and survey readiness and remediation.  Her prior experience includes serving in a variety of roles in home health and hospice from field nurse to executive director of home health and hospice. She also served as the community integration executive director for a health system in Maryland focused on building collaborative partnerships with community providers including skilled nursing facility, assisted living facilities and physicians to improve population health and reduce hospitalizations.  She was a surveyor for home health, hospice and private duty services for CHAP (Community Health Accreditation Partners).  Diane is a frequent contributor to Home Health Line/Decision Health articles and is known for her inspiring and knowledgeable presentations at state and national conferences.  She is the author of The Hospice Guide to Quality Care and Reporting: Promoting Sustainability in an Evolving Regulatory Climate. Diane has a Master’s Degree in Healthcare Administration and a green belt in Lean Six Sigma from Mount Saint Mary’s University in Maryland.
